FAQ

What does True North Transitions help with?

We provide administrative support for MassHealth long-term care applications for nursing home eligibility in Massachusetts.

Do you provide legal advice?

No. We are not a law firm and do not provide legal advice or legal representation. If your situation requires legal planning, we may recommend that you speak with an elder law attorney.

How quickly can we start?

Often, we can begin right away with a free 15-minute clarity call to understand the situation and outline next steps.

What information should we have ready for the first call?

Do not worry if you do not have everything yet. Helpful information may include the applicant’s name, current care setting, where things stand in the admission process, and any letters already received from MassHealth.

Can you talk to the nursing home or other parties for us?

We can coordinate with authorized parties when proper permission is in place.

Can you guarantee approval?

No. MassHealth eligibility decisions are made by the state, and approval cannot be guaranteed.

Do you help with short-term rehab insurance questions?

Our primary focus is MassHealth long-term care application support for nursing home eligibility. If a short-term rehab stay is shifting toward long-term placement, we may be able to help with the next steps related to the MassHealth application process.

Do you help people remain at home?

Our main focus is nursing home eligibility applications. In some situations, we may help families understand when a home-based support program or another pathway may need to be explored, but our core service is MassHealth long-term care application support.
